App is blank when try to open downloaded one on Android phone

I made app com.nrai . Got the confirmation link on my email. Installed it but it is showing nothing. The app has a form and youtube video. Please suggest where the problem is

You didn't select the modules for the builder, you need to enable the module with a "check mark" before you build. Every build has a different APK and you need to re-install.
